Application Process Instructions
Please read through the application and answer each question that applies to you. Answer each question completely and honestly. Fill the application out yourself and remember that no one may stay at Well of GRACE Ministries against her will or without showing a desire to work hard in addressing her problems while living at Well of GRACE Ministries. The average stay is estimated to be between 6 and 9 months with a minimum of 30 days. If there is information you desire that is not included in this application package please call Well of GRACE Ministries and ask for the In-Take Team.
Remember that a sincere desire to overcome the issues you are facing is of the utmost importance.
Review the application package and if you are in complete agreement with the rules and policies complete the application yourself.
Fill out the application completely. Information that is omitted without explanation will slow the process down or we will not process it at all.
Make sure you send along the signed Release of Information Form(s) with your application.
Mail the application to: Well of GRACE Ministries, 1672 Roberts Dr, St. Joseph, MI 49085, ATTENTION: IN-TAKE TEAM.
You must call Well of GRACE Ministries to make sure your application was received. Well of GRACE Ministries will not call you. After mailing your application, please wait three days to be sure we have received it before calling. When you call to verify that we have received your application we will schedule an interview at that time.
After reviewing your application, the In-Take Team will make a decision regarding acceptance to our program or the appropriate referral.
If accepted into our program you will be instructed to complete physical health forms and to make doctor's appointments for examinations before entering Well of GRACE Ministries.
An entrance date will be given at this time, provided we have received all information requested. If there are no rooms available, you will be placed on a waiting list.
